New Resources Added to Online WI CCC Plan

A new resource has been added to the online WI CCC Plan resource section. It is a series of toolkits designed by George Washington University titled “The National Cancer Survivorship Resource Center Toolkit”.  You can access resources for specific Priorities and Strategies when viewing the WI CCC Plan 2015-2020 online. Select the Priority you wish to explore and scroll to the bottom right of your screen. Under “Resources to take action” click on the Strategy to find resources to support your implementation activities.

  • Implementing Clinical practice Guidelines for Cancer Survivorship Care: This resource can be found when referencing Priority 9, Strategies A and B, in addition to Priority 10, Strategy A. This tool aims to help implement clinical practice guidelines for cancer survivorship care.
  • Cancer and Public Health Official’s tools: This resource can be found when referencing Priority 9, Strategies A and B, in addition to Priority 10, Strategy A. This tool can be used to help ensure cancer survivors receive evidence-based follow up care.
  • Patient, survivor, caregiver, and family member tools: This resource can be found when referencing Priority 9, Strategies A and D, along with Priority 10, Strategies A and B. It is aimed at assisting patients, their families and caregivers, when talking with physicians regarding cancer care. It outlines important topics pertaining to survivor care.
