The WI Comprehensive Cancer Control Program is delighted to share the 2015 recipients of the CCC Implementation Grants. This year’s grant program was very competitive, with nearly 40 applications received. The intent of the CCC Implementation Grants is to fund local projects that address cancer prevention and control efforts in Wisconsin, linking directly to the priorities or strategies in the WI CCC Plan 2015-2020. The projects began in April 2015 and will be funded through October 2016. See below for more information regarding the six projects funded by the 2015 CCC Implementation Grants Program. Indianhead’s Head Start Tobacco Cessation Initiative
Organization: Indian Head Community Action Agency
Priority 1 – Decreasing tobacco use and exposure
Indianhead Community Action Agency is working to decrease tobacco use and exposure among their Early Head Start/Head Start families through education about the dangers of tobacco and targeted interventions. Agency staff will be trained on how to engage parents in discussions through motivational interviewing and will help them set smoking cessation goals.
Fruit and Vegetable Rx
Organization: Agnesian Cancer Center
Priority 2 – Increase healthy, active lifestyles
All newly diagnosed breast cancer patients at Agnesian Cancer Center will be screened for obesity and, if appropriate, given the opportunity to enroll in the Fruit and Vegetable Rx program. Participants in the six month program will receive a weekly box of fresh produce along with support and education from a registered dietitian.
Reducing cancer risk disparities among LGBT People in Wisconsin
Organization: Diverse and Resilient, Inc.
Priority 2 – Increase healthy, active lifestyles
Lesbian, gay, bi-sexual and transgender (LGBT) individuals face a number of health disparities. Diverse and Resilient, Inc. is committed to addressing these disparities through outreach, education and the creation of a new resource –
LGBT Passport to Health – that will include information about risk factors, healthy living, cancer screening recommendations and more.
Community Support for HPV Vaccine Completion
Organization: Shawano-Menominee Counties Health Department
Priority 4 – Increase HPV vaccine completion
The Shawano-Menominee Counties Health Department project has two goals – to increase knowledge and acceptance of the HPV vaccine among teens and their caregivers and also increase access to the HPV vaccine. A comprehensive promotional campaign will be used to reach youth, especially boys, and their parents.
Healthy Hmong Families: Advancing Equity through Lay Health Education
Organization: Milwaukee Consortium for Hmong Health
Priority 7 – Increase use of recommended cancer screenings
The Milwaukee Consortium for Hmong Health is dedicated to increasing awareness of and access to recommended cancer screenings among the Hmong population. Community Health Workers and Youth Health Messengers will be used to educate and encourage cancer screening within the community.
Advance Care Planning in Independent Critical Access Hospitals
Organization: Honoring Choices Wisconsin
Priority 11 – Increase advance care planning
Honoring Choices Wisconsin, an initiative of the Wisconsin Medical Society, is working to overcome the barriers that independent rural hospitals face to implement advance care planning. Honoring Choices is working with four additional hospitals to implement the Honoring Choices Wisconsin program and will develop a regional support network in southwest Wisconsin.
