New resources have been added to the online WI CCC Plan “Resource” section. You can access resources for specific priorities and strategies when viewing the WI CCC Plan 2015-2020 online. Select the priority you wish to explore and scroll to the bottom right of your screen. Under “resources to take action” click on the strategy to find resources to support your implementation activities.
- CancerCare Resource: This resource can be found when referencing Priority 9, Strategy B. CancerCare serves as a tool to help identify counseling, support services, education, and financial assistance to cancer patients.
- Patient Navigation Barriers and Outcomes Tool (PN-BOT): This resource can also be found when referencing Priority 9, Strategy B. This tool aims to assist in data entry and management and reporting designs for cancer patient navigation programs nationwide.
