LLS – Clinician Strategies and Resources II: Bridging Primary Care and Oncology in AYA Cancer Survivorship

On September 8th, 2025 from 12-1 PM, the Leukemia & Lymphoma Society is hosting a live webinar with continuing education credits.
This activity is intended for hematologists/oncologists, advanced practice providers, nurses, psychologists, social workers, and other healthcare professionals involved in the care of adolescent and young adult (AYA) survivors of cancer. This webinar, featuring a panel of subject matter experts, explores the unique challenges and opportunities to support adolescent and young adult (AYA) survivors of cancer. The role of primary care clinicians in managing long-term follow up care, strategies to strengthen communication between oncology and primary care teams, and resources to support AYA survivors, their caregivers, and healthcare professionals will be discussed, with opportunities for input from the audience.
Learning objectives include:
  • Describe the unique survivorship challenges AYA patients face during care transitions and integration with primary care
  • Explain the important role of primary care clinicians in delivering long-term follow up care for AYA cancer survivors
  • Apply communication and care coordination strategies to strengthen the relationship between oncology and primary care teams
