Honoring Choices Wisconsin: "Advance Care Planning in Independent Critical Access Hospitals"
Grantee: Honoring Choices Wisconsin, an initiative of the Wisconsin Medical Society, was a recipient of a 2015 CCC Implementation Grant. Honoring Choices Wisconsin is committed to “promoting the benefits of and improving the process for advance care planning” in health care settings across the state.
Project: “Advance Care Planning in Independent Critical Access Hospitals” was designed to improve advance care planning conversations in a cohort of rural hospitals to increase the advance care planning conversations among rural cancer patients.
Goal: The overarching goal in this project was to increase support for, and execution of, advance care planning in rural Wisconsin communities.
Results: Through the development of ready-made materials, 51 people from three rural hospitals and one managed care organization received education on designing an advance care planning system and 28 facilitators were certified to have advance care planning conversations.
What’s Next: Honoring choices plans to expand in additional rural counties.
