Action Steps

  • Facilitate ACP conversations with all cancer patients to develop an advance care plan early in their treatment.
  • Develop models/algorithms for reviewing patient advance care plans throughout the cancer experience.
  • Integrate advance care planning into survivorship services at the beginning of treatment.
  • Educate cancer providers and the community that advance care planning is an opportunity to empower cancer patients in setting goals of care.

Action Steps

  • Ensure that ACP documents are consistently and reliably stored in electronic health records and accessible within and across medical settings.
  • Support efforts to improve electronic health records and patient charts to include easy access to ACP documents for providers and patients.
  • Support a state wide effort to promote a “pocket/wallet card” with information on which health system stores an individual’s ACP documents and identifies their health care agent.
