SUMMARY:Advance Care Planning: It's About the Conversation
DESCRIPTION:This one-hour session will teach participants about advance care planning. Advance Care Planning is a process of understanding\, reflecting on\, and discussing future medical decisions regarding of age and current health status. All adults should think about\, discuss and create an advance care plan. Having an advance care plan is truly a gift for yourself and others.\n\nThe session will cover:\n– What is advance care planning\n– Understanding your healthcare treatment options\n– Clarifying your healthcare goals\n– Weighing options about the kind of care you would want or not want\n– Why an advance care plan is important\n– How to create an advance care plan\n– How to communicate your wishes with your family\, friends and healthcare provider\n\nAdditional resources and a small incentive will be available for participants following the webinar. This webinar is presented by GrapeVine – a program of the Wisconsin Women’s Health Foundation\, which trains nurses to lead women’s health education in their Wisconsin communities.
